PDPF: Adult Protective Services Investment (APSW)

ABLE2, in partnership with Developmental Service Ontario – Eastern Region (DSO-ER) and the Ontario Ministry of Children, Community and Social Services (MCCSS) offers a program to support adults with a developmental disability who would like to move to a home of their own.

Moving to your own place is an overwhelming process and it can be difficult to know where or how to start. The APSW program will connect you with a Worker who will work with you to identify your goals, develop a plan and work through any issues or barriers

To start you must contact Developmental Service Ontario – Eastern Region (DSO-ER) or your DSO-ER case worker to determine your eligibility for the program.

The APSW Program can support you to:

  • Identify your housing goals, and build a plan that addresses your wants, needs, strengths, areas of support, where you want to live and what your home may look like and who will help you (friends, family, professionals and other service providers). 
  • Finalize your plan and put it into action. 

Your APSW Worker will: 

  • Meet with you regularly. 
  • Provide information, referrals and coordinate community resources.
  • Help you with any barriers or issues you may encounter.
  • Provide information and referrals, and coordinate with community resources.

This program is offered in both official languages.

What is a housing plan?

A housing plan is developed by you and your APSW Worker. This individualised plan identifies your housing goals, and how to achieve those goals. It addresses your wants, needs, strengths, areas of support, where you want to live and what your home may look like and who will help you (friends, family, professionals and other service providers). 

This plan is a guide for action and can be changed as needed. It will help you to figure out when and if you ready to move into your own home. You are in control of when everything happens with the plan, and it can be updated as circumstances change.  

I’m interested but what if I don’t know if I’m ready to move? How will I know?

We will help you to identify your needs and wants. 

What if I want to move in a few years from now?

We can still help by helping you to identify your goals and set up a plan.

How do I find a place to live?

Your Worker and anyone you wish to be included in your plan can support you to find your home.

I don’t know how to do a lot by myself, how can I live on my own?

That’s ok. No one knows how to do everything on their own the first time. Your Worker can help you develop the skills you need to live on your own, or work with you to find supports in the community that can help you.

But I will never be able to afford a place of my own – what then?

Your Worker does not have all the solutions, but other than developing a plan with you, they can also help to find resources in the community that may help to achieve your goals.
